Hospital & Acute Support
In addition to community-based care, Independent Pathways Care provides skilled support workers to hospital settings. When individuals with complex needs are admitted to hospital, they often require familiar, specialist support to ensure their safety and wellbeing.
We can deploy experienced staff to work alongside hospital teams, providing:
- 1:1 or enhanced observation and support
- Behaviour management in acute settings
- Familiar faces to reduce distress and aid communication
- Continuity between community and hospital care
- Support with discharge planning and transition back to the community
